Scholarships and Awards Application Forms and Resources


This page has been divided into four sections:

All forms are in Adobe's Portable Document Format (PDF) and you will need the free Adobe Acrobat Reader application installed on your computer in order to view them. These forms can be printed and filled out manually or completed directly on your computer by typing the necessary information, then printing.

Because duplicates and signatures are required for processing, the forms cannot be submitted online at this time.

2011-2012 Award Payment Schedule: Awards Paid Through Student Accounts

Award payments will occur on a scheduled basis throughout each terms. The schedule below shows the earliest payment date per term. Award payments will be applied to your student account first, paying off any outstanding tuition balance. The remaining funds will be transfered to your bank account. Please note that there is a 2-3 day delay in the funds being transferred to your bank account.

To receive payment students must be registered and have submitted the required documentation to Dean of Graduate Studies. The required documentation includes the signed Award Acceptance Form and the completed Direct Deposit Form.

 

The schedule below applies to the following awards only;

a. Federal: NSERC IPS (NSERC portion only), CIHR Doctoral and Student awards, CGS-MSFSS (Foreign Study Supp)

b. Provincial: MSFHR, Pacific Leaders, PICS

c.  All private awards

d. All donor-designated (external) awards

e. Graduate Fellowships (GFs)

Please note: We only have your banking information if you have held an award with us. We do not have your banking information if you were paid through payroll for Teaching Assistantships or Research Assistantships. (For privacy reasons, we do not receive banking information from payroll.)

Tuition waivers: If you have a tuition waiver and an award, your award will be processed first which will pay off your tuition. Tuition waivers are generally processed in the fifth week of class, after which you will need to complete and submit a Refund Request from a Student Account Form to Student Accounts in Student Services.

Award Payment Activation for External Awards

 

SSHRC/CGS Doctoral or Masters Award

Did you receive notification that you were awarded a SSHRC CGS Doctoral or Master's Award? Here is what you need to do in order to activate your award:

NSERC/CGS Doctoral or Masters Award

Did you receive notification that you were awarded an NSERC CGS Doctoral or Master's Award? Here is what you need to do in order to activate your award:

Send the Dean of Graduate Studies Office (c/o Awards Assistant) the following:

Send directly to NSERC the following:

  • The completed Payment Activation Form Part II
  • Other forms from the above page as required (Request to Change University, Request for Deferment, etc). The Dean of Graduate Studies Office would greatly appreciate a copy of these forms if relevant.

 

CIHR CGS Masters Award

Did you receive notification that you were awarded a CIHR CGS Master's Award? Here is what you need to do in order to activate your award:

Send the Dean of Graduate Studies Office (c/o Awards Assistant) the following:

  • a copy of the CIHR Offer of an Award, which includes the award name, start date, duration and value
  • a copy of the CIHR Response to an Offer of Award
  • CIHR Confirmation of Commencement, signed by award-holder and supervisor
  • CIHR Master's Awards Confirmation of Registration Form. Award-holders are required to submit this form to DGS for completion - the completed form will be sent to CIHR on your behalf.
  • SFU Payroll's Personal Data Form 
  • A voided cheque
